Once you’ve created a form in Axial Shift, the next step is to make it accessible to your team by adding it to a specific logbook channel. Follow the steps below to easily attach your form where it’s needed most.
Step-by-Step: Adding a Form to a Logbook Channel

Navigate to Settings
Hover over the Settings menu and click on Staff Note Types.
2. Locate Your Form
You’ll see a list of the forms you’ve created. Find the one you want to use and click the pen and paper icon to open it.
3. Assign to Specific Channels
Click on Restrict to specific channels.
From the dropdown menu, select the channels where you want this form to be available.
4. Save Your Changes
Once you’ve selected the appropriate channels, click Save.
Accessing the Form in the Axial Shift App

Open your Axial Shift app and go to the Hub.
2. Choose the channel you assigned the form to.
3. Click on the paperclip icon (or plus sign) to add a new entry.
3. Select Add New Staff Note Entry.
4. You’ll now see the form you just added listed and ready to use!
